  • What should I bring to my visit?

    Any past vaccine records (must be in English). If you have vaccination records in another language, you may be advised to take blood tests or vaccines.

  • How do I fill out the paperwork I need?

    After you check in, you need to fill out patient information sheet. This sheet can be obtained under the "Forms" Quick Link.

    Our medical assistant will fill out the first page of the I-693 form and check your vitals. Civil surgeon will review your medical history and perform an exam pertinent to immigration

  • What labs and vaccines do I need?

    Vaccines and labs that at are mandatory for immigration physical exams will be reviewed. 

    Vaccines may be given at the exam if necessary. Labs are to be completed after the visit. The doctor will determine which labs are necessary for you during the examination.

  • How do I get my completed paperwork?

    You can choose to pick up your completed form at our office or we can mail it to you at additional cost. 

    If you would like a friend or spouse to pick up your paperwork, you need to give us an authorization to release your information. 

    You must not open the sealed envelope that says “USCIS use only.” That should go directly to USCIS or your lawyer.

  • Will Insurance cover my appointment?

    Insurance does not cover the Immigration physical exam.

Our goal is to make this process as easy for you as possible. Please let us know if you have any special requests. If you need a translator, we have staff who can speak Spanish, Portuguese, Brazilian, Telugu, Hindi and Marathi. If you have more than one person in the family needing an immigration medical exam, let us know so we can book you in back-to- back appointments.
